Travel Approval Form (pdf): required before traveling. Division head and chair’s approvals are required; email approvals are sufficient.

CTA Form (pdf) (if applicable): required for pre-travel arrangements by the Department (flight, hotel).

PI Dashboard: Additional tools accessed on the PI portal of the dashboard include a self-inspection tool, a staff list, and lab equipment records.
The dashboard also shows the safety rating calculated from the inspection results.
