Travel

• Travel Approval Form (pdf): required before traveling. Division head and chair’s approvals are required; email approvals are sufficient.
• CTA Form (pdf) (if applicable): required for pre-travel arrangements by the Department (flight, hotel).
• Travel Reimbursement Form (pdf): required post-travel to keep track of expenses for UW employees. 
• Non-UW Employee Travel Reimbursement Form (pdf) (if applicable): required post-travel for non-UW employees (no need to fill & submit the DLMP travel reimbursement form; this will replace that for non-UW travelers only).
