Wound Aerobic and Anaerobic Bacterial Culture and Sensitivity with Gram Stain
General Information
Lab Name
Wound C/S w/Gram (Anaerobic)
Lab Code
WNDANC
Epic Name
Wound C/S w/Gram (Anaerobic)
Description
Specify exact source of specimen.
This culture is intended for abscess, hardware, deep and/or sterile wound sites, or when rule out anaerobes is required. To rule out possible anaerobes that commonly cause skin infections swabs from boils, furuncles, and cysts will be reordered and processed as a WNDANC.
If rule out Actinomyces is required as well as routine culture, order additional separate culture R/O Actinomyces Bacterial Culture and Sensitivity [ACTINC].
Superficial wounds (skin and superficial sites such as ulcer, superficial abrasion, first degree burn, impetigo, folliculitis, and cellulitis), see Wound/Skin Aerobic Bacterial Culture and Sensitivity with Gram Stain [WNDC].
To rule out S. aureus only, see R/O Staph aureus Bacterial Culture only without Gram [STAPHC].
Body fluids or drains, see Place Holder for Body Fluid Bacterial Culture and Sensitivity with Gram Stain (includes anaerobes) [FLDPHC].
Tissues or biopsies, see Tissue Bacterial Culture and Sensitivity with Gram [TISC].
Genital cultures, see Genital Bacterial Culture and Sensitivity (Routine) without Gram Stain [GUC] or Genital Bacterial Culture and Sensitivity (Anaerobic) with Gram Stain [GUWANC].

Guidelines
Ordering & Collection
Specimen Type
Collection
Disinfect skin and remove surface exudate before collecting specimen to minimize contamination.
- Aspirate: abscess fluid in syringe. Remove needle, expel air and cap syringe.
- Swab: E-swab is preferred. Gel swab is acceptable. (DO NOT remove the liquid media in the E-swab.)
- Hardware or devices in sterile containers
Unacceptable collections will be rejected for culture:
- Specimens collected in Universal Transport Medium (UTM) or formalin.
- Syringes with needles attached.
- Specimens collected with dry swabs or expired transport media.
**Specify the exact source and/or site of specimen and "R/O Anaerobes" for optimal workup.
Handling Instructions
Abscess fluid: Transport in capped syringe, ideally within 2 hours of collection, but not more than 24 hours after collection.
Swabs: Transport at room temperature, ideally within 4 hours of collection, but not more than 24 hours after collection.
Specimens will be rejected if received > 24 hours after collection. Viability of organisms is significantly reduced after 24 hours.
Note: Cold temperature reduces the viability of anaerobes.
Quantity
requested: Eswab or as much fluid as possible
minimum: Eswab or 1 mL fluid
Processing
If specimen is described as "superficial" or "skin" and rule out anaerobes is not requested, log in WNDC.
- Exception: Boils, cysts, furuncles, and drainages will be relogged as WNDANC for optimal workup.
If there is no site given, process as a WNDANC. Additional site information may be inquired about.
